    Hi Wellmedic team

    I just wanted to clarify on the DS1500 whether we have to put the prognosis or not. seems gov.uk website states not to put a prognosis . it says The form asks for factual information and does not require you to give a prognosis.

    please clarify

    Hi Andrea,

    This is correct. Sorry for the confusion, for the purpose of these benefits, someone is considered terminally ill if they have a progressive disease and the person’s death in consequence of that disease can reasonably be expected within 6 months.

    Including an absolute time-frame is not required. If a patient does exceed the 6 months, the benefits may be re-addressed at three years.
